You are doing a great job maintaining your dental health, keeping your periodic check-up and cleaning appointments! With this program, not only will your smile remain healthy, we can also make it shine!

The cost is a one-time enrollment fee of $99.00 - a savings of $350 off our normal fee! Below are the program specifications:

• Custom Bleaching Trays. You will receive bleaching trays that are custom fitted to your mouth - normally a $450.00 value.

• Keep your preventive visits current. Whether your cleaning appointments are every 3, 4 or 6 months, maintaining your long term dental health is as important to us as it is to you. In order to remain in the program you need to keep your regular appointments. A result of doing so, our patients have greatly reduced the need for emergency visits.

• At each preventive visit you will receive a touch-up syringe. Additional syringes are $10.00 each.

• We ask that you provide at least 48 hours notice if you need to change an appointment. In order to provide exceptional services like Your Healthy Smile Program to all of our patients, we ask that you give us the courtesy of advanced notice for schedule changes. If you do not show up for your appointment or fail to notify us that you are not going to make your appointment, your program will automatically cancel. If you'd like to be re-instated, an additional fee of $25.00 per occurrence will be charged.

• If necessary, we will provide Desensitizing treatment should you develop sensitivity, which occasionally patients will develop during the use of their bleaching trays.
